Healthcare providers and pharmacies participating in your care may be covered by federal and state health privacy laws. Each participant is responsible for its own legally required privacy notice. This page is a general summary and is not a substitute for those notices.

Uses and disclosures

Health information may be used or disclosed for treatment, payment, healthcare operations, public health and safety activities, oversight, legal requirements, and other purposes permitted by law. Other uses generally require authorization.

Your rights

Depending on applicable law and the organization holding your record, rights may include requesting access or copies, corrections, restrictions, confidential communications, an accounting of certain disclosures, and a paper copy of the operative privacy notice.

Complaints

You may complain to the organization responsible for your record or to the U.S. Department of Health and Human Services. Retaliation for filing a privacy complaint is prohibited.

For provider records, contact LocumTele using the instructions provided during care. For pharmacy records, contact Rush Pharmacy. Before launch, Medora Health must add verified privacy-officer contact information and the operative relationship among all participating entities.
